The first time you stumble across an altar, all characters in your account will benefit from a permanent enhancement of their attributes.

Altars of Lilith: Benefits
Altars of Lilith have more to offer than you might think. They offer a variety of attribute bonuses ranging from increased Dexterity and Strength to an expanded Murmuring Obol’s capacity and even an additional Paragon Point. If you can find all 160 Altars in the game, your characters will receive a massive boost to several attributes. This gives you an edge.
The Altars of Lilith Locations in Diablo 4 are a fun scavenger-hunt. Altars can be found in a variety of terrains, from the obvious to the cleverly hidden. You’ll start the game with one altar. As you progress through the first area of the game (Fractured Peaks), you may find up to 28 Altars.
As you reach Kyovashad (the central city), you’ll discover more. Some of these are hidden within structures, or hidden paths. Others are within strongholds, which require clearing the area. Each of the five regions in Diablo 4 has their own set of Altars.
